A Cataract Surgery System

Over 37 years as an ophthalmic surgeon, I have learned a lot about cataract surgery.

I started learning phaco cataract surgery back when it was first introduced into ophthalmology practice in the late 1980s.

I had been in private and public hospital as an ophthalmologist for about 5 years doing routine large incision extracapsular surgery. 

I taught myself the new phaco small incision technique taking one baby step at a time and performing the rest of the procedure in my usual large incision extracapsular way. 

I learnt the easiest steps first. 

I learned the steps from watching videos of the procedure and visiting surgeons in their day surgeries who were already competent in the procedure. 

I ended up over my career performing more than 35,000 phaco cataract surgeries.

I’ve always been passionate about teaching. 

I was forced to take off 5 years from practicing clinical ophthalmology due to a severe neck injury caused by poor posture examining patients on the slit lamp and performing retinal laser for long periods. 

During this time, I completed a PhD in medical education and had academic roles teaching medical students at Monash University. 

I was also Director of IMG assessments at the RACS (Royal Australasian College of Surgeons) assessing overseas trained surgeons regarding their comparability to practice in Australia. 

Throughout all my years in practice I have mentored and trained ophthalmology trainees initially in the Queensland RANZCO training program at Royal Brisbane Hospital for 7 years and then later in the Victorian RANZCO training program at the Royal Victorian Eye and Ear Hospital in Melbourne, for over 20 years.

I decided to develop a framework for teaching every aspect of cataract surgery in a systematic way to make you as the surgeon, feel confident, relaxed and focused doing cataract surgery. 

The system is a framework so you will be prepared for every eventuality from complex cases to managing complications well, together with improving your technique for the regular cataract cases.

I’m on a journey and I want to take you with me. Are you interested in coming?

Back when I first received my FRANZCO I wanted to practice as an accomplished, relaxed, confident ophthalmologist and I wanted to be a success.

But I didn’t know what that really looked like. 

My career ahead seemed like a black box and I wasn’t sure which way I was going.

I started out as a general ophthalmologist in Brisbane, Queensland and I was invited to join a prestigious group of 6 other ophthalmologists, all highly respected.

Our “rooms” were on Wickham Terrace, Brisbane which is where all the specialists had their own consulting rooms.

I wondered how would I get any patients to come and see me?

Or would I simply just get the “leftovers” from the other established ophthalmologists who were my associates?

I secretly feared that even though I had completed the RANZCO training program and passed all the requisite examinations, 

what if I wasn’t really fully prepared for any eventuality?

What if I got complications in my private practice operations?

In particular what if I broke a lot of posterior capsules or got a dropped nucleus? 

Or worse, an endophthalmitis?

How was I going to learn now in private practice how to deal with all these rare occurrences?

The only way available at the time was serendipity, asking colleagues, going to international meetings and reading journals. 

The serendipitous part was which patients arrived at my practice and how I treated them or operated on them.

I never knew what I was going to get or see next. It was a minestrone soup again just like my training program.

I also worked part time at two public hospitals in Brisbane, 

The Royal (RBH or Royal Brisbane Hospital) and 

Greenslopes (The Repatriation Hospital, Greenslopes) serving the Veterans.

I never knew what I was going to get at the public hospital either,

but I knew whatever it was, it was going to be a lot worse than what I saw privately.

I was right. I learned far more from my public hospital appointments for two reasons:
 

1. The patients were sicker and the cataracts were worse than in private and
 

2. I was teaching the trainees so I always had to know more than them, and I learnt more than they did, from managing the cases.


 

What I learned from by chance or serendipity in my early career has put me on my current journey to create an alternative to learning all the nuances of cataract surgery.


 

I am on a journey now to create a comprehensive system to learn cataract surgery.
 



It’s the resource I wish I had had at that early stage of my career.
 

If I had it then I would not have taken all the years it took to become a relaxed, confident and successful cataract surgeon.
 



I would have been able to fast track that outcome and have experienced far less stress.
 



My new system has 6 Modules:

3. Managing Complex cases with confidence and expertise.
 

4. Prevention and management of complications in cataract surgery.

6. Looking after the surgeon for a long and successful career.
